As someone who works with stem cells from time to time, and who has colleagues who work with induced pluripotent cells, the most baffling thing about the STAP debacle to me was that anyone believed it in the first place. I took one look at that paper and rolled my eyes back when it was first published. The power of wishful thinking his strong.

The line between fraud and (unintentionally?) skewing an analysis to favor making headlines is a fine one. Edit: I would argue that it does all boil down to intention, which isn't always crystal clear.
