I've noticed that if no one says anything, then NO ONE says anything. However if just one person says something others follow.
I live in SF and when I see something like this I stop and ask them if they need help, then I call the police. Whenever I do this others stop and get involved.

This is such a great idea. The closest thing we have where I live is a time bank but those require too much of a commitment.
