Well, as a very young boy, I always seemed to really like Roxy Music's Country Life album cover for some strange reason: http://imgur.com/2pt2R (NSFW).
But the first album cover that truly excited my mind was probably Joy Division's Substance: http://i.imgur.com/UaT0G.jpg (SFW).
