It must be incredibly frustrating, especially when the solutions are obvious. Accountability seems to me like the biggest problem here. All of the mentioned quirks, issues and actual deaths seem to stem from it. Any semi-decent public transport organization would have board members resigning when preventable and critical issues weren't prevented and resulted in issues like fires, crashes and death. Politics, what is it good for...One explanation for Metro’s perpetual troubles lies in what could be considered its original design flaw: Responsibility for the agency is divided among so many players that ultimately no one is held accountable.
